Personal Auto Medical Services Billing

If you are a patient who received a medical bill, please contact the provider with your NJM claim number to avoid a delay in processing.

Request a copy of an Explanation of Benefits (EOB):

EOB request process change, effective 6/1/2024 — For information security reasons, if services were submitted electronically, you must contact the provider’s clearinghouse to obtain a copy of an EOB.

For services rendered in New Jersey, you are required to access EOBs through the Jopari Portal.

  • If you do not have login information, please contact your client for access information.

If your services were rendered outside of New Jersey, you must provide a letter of authorization from the medical provider for NJM to release your billing and payment information. The letter must be:

  • Submitted on the medical provider’s letterhead
  • Dated within 12 months of the request
  • Signed by the medical provider’s representative

You may send a written request along with the letter of authorization to EOBRequests@njm.com. Please include the following information:

  • Claim number
  • Patient name
  • Provider name and TIN
  • Date of service
  • Billed amount
  • Return email address

Requests that do not include a letter of authorization will not prompt a response.

If you represent a medical provider’s office and are interested in electronic billing, contact NJM Provider Services at 1-800-232-6600, ext. 6020, or Jopari Solutions Inc at 1-800-630-3060. Workers’ Compensation Payor ID is 12122 and Personal Auto Payor ID is C1048.
